Why didn't Ryan hear what was going on?
For new players, the band is thought of as "The Band," an entity that just happens and the thought process that they may be off is a distant one. Up to this point most players have commonly only played to jam tracks, which are infallible. It's important to listen to the form to make sure you're hearing what you expected/requested.
In this case, Ryan asked for "From the V" and would be listening for chord change every bar and for a turnaround at the end of the bar 12. In this song there is no turnaround (V7 at the last couple beats of bar 12), so the first three chord changes, one per bar, are important.

Not knowing he needed to listen for those changes, he just waited four bars and did his thing... unfortunately the band didn't do their thing and forced him to be off by four bars of the form.
